Our team of Support Coordinators will assist you to use the funding in your plan to achieve your goals, develop and increase your skills and capacity to live independently at home, in the community and in employment. Support coordinators aim to support participants in developing greater knowledge, experience and connections within the community and to develop a wider system of support. They work closely with the participant, their family and care team to understand the funded plan and its purpose.
They develop and design the support solutions to achieve the participants goals. They understand the participants skills and confidence and allow participants to identify what they want from their services. The Support Coordination Team also helps the participants and their family to identify and consider support options and link them to the broader systems of support.
They help the participants to implement their plan to achieve their goals and assist them to resolve points of crises and develop capacity and resilience in the participant’s network. They also coach the participant through challenges that may arise in future and help to prepare for plan reviews and reports on achieved outcomes.
